Recovery after breast augmentation is often the part patients think least about before surgery, and the part they think about most afterwards. If you have chosen the Motiva Preservé programme at iQonic Aesthetics, you have already opted for an approach designed to work with your body's tissue rather than against it. That philosophy does not stop when you leave the operating theatre. It carries through every stage of your recovery.
This guide walks you through what to expect week by week, what to watch for, and how to give yourself the best chance of a smooth, lasting result.
Most patients return to desk work within 2-3 days and resume light exercise after a few weeks. Full recovery, including scar maturation, takes approximately three to six months. The tissue-preserving technique used in Motiva Preservé reduces disruption to the breast parenchyma (the functional glandular tissue of the breast) and pectoral fascia (the connective tissue layer over the chest muscle), which generally means less postoperative swelling and a faster early recovery than conventional subfascial or subglandular approaches.
Understanding the technique helps you understand your recovery. Standard breast augmentation often involves significant dissection of the breast tissue and surrounding structures. Preservé takes a different approach: the dissection is limited, the natural tissue planes are respected, and the implant pocket is created with precision instruments that minimise trauma to nerves, blood vessels, and the glandular tissue itself.

The first three days set the tone for the weeks ahead. Your body is managing inflammation, and your job is simply to support that process.

You should sleep on your back with your upper body elevated at roughly 30 to 45 degrees. A wedge pillow or two to three stacked pillows will do this. Staying elevated reduces blood pooling in the breast tissue and keeps swelling lower. Avoid rolling onto your side during the first 72 hours.
Short, gentle walks around the house starting on the day of surgery reduce the risk of deep vein thrombosis and help circulation. Keep your arms close to your body and avoid lifting anything heavier than a cup of tea.
Your surgical dressings will be applied in theatre. You will leave the clinic in a post-operative support bra specified by your surgical team. Do not remove or adjust this yourself. Your nurse will give you clear instructions about when and how to change dressings at your first post-op check.
Take your prescribed analgesia on schedule for the first 48 hours rather than waiting for pain to build. Ibuprofen and other non-steroidal anti-inflammatory drugs are typically avoided in the early post-op period unless specifically prescribed, as they can affect platelet function. Your team will clarify this at discharge.
By day three to four, most of the any tightness begins to ease. Swelling remains but should feel stable rather than worsening. You will attend your first post-operative review at the clinic, where your dressings are assessed, your wound edges checked, and any questions addressed.

Swelling continues to reduce. Your implants will sit higher than their final position at this stage. This is entirely normal. The lower pole of the breast softens and drops over six to 12 weeks as the pocket relaxes. For most people, week two is when the visible bruising fades and the early result begins to feel real.
Your stitches, if not dissolvable, will be removed at your second post-op review. Your scar care programme begins at this appointment.
A secondary support bra replaces the immediate post-operative garment. Wear it consistently, day and night, unless instructed otherwise.
Exercise clearance after Preservé is structured to protect the tissue integrity that the surgical technique worked to preserve. Returning too early to high-impact or upper-body exercise is one of the most common reasons for compromised results.
The following timeline reflects standard guidance. Your surgeon will confirm your personal clearance at follow-up appointments:

You can transition to sleeping on your side once your surgeon confirms comfort and stability, typically from week 4 to 6. Avoid sleeping face-down for three months.
Alcohol should be avoided for at least the first two weeks, and ideally the first month. Smoking significantly impairs wound healing and increases the risk of capsular contracture and infection. If you smoke, you will have been advised to stop before surgery. Continue to abstain for at least six weeks post-operatively.
Short car journeys are fine from week two. Long-haul flights are generally not recommended before six weeks due to DVT risk and cabin pressure effects on swelling. Discuss any travel plans with your clinical team before booking.
The Preservé incision is in the inframammary (in the fold beneath the breast). Preservé technique allows for the smallest 2.5-3cm scar is in a low-visibility position and, with proper care, becomes barely perceptible over 12 to 18 months.
Your nurse will introduce your scar care regimen at the two-week post-op review once wounds are confirmed healed. Standard evidence-based options include silicone gel or sheeting, applied consistently twice daily, and SPF protection over the scar when exposed to sunlight for the first 12 months.
You should not apply any scar product until your nurse confirms the wound is fully closed.
Your final result is not visible at week two, or even week six. Full implant settling takes three to six months as the pocket matures and the breast tissue relaxes over the implant. Upper-pole fullness that looks high early in recovery is normal and resolves with time.
